The Tooth Taxi is a 39 foot state-of-the-art mobile dental van that visits public schools and neighborhood organizations serving children and young people in pre-kindergarten through twelfth grade throughout Oregon. Our goal is to reach children and young people with limited to no access to dental resources, and students without insurance. However, when partnering with schools, we will help any child whose parent fills out our paperwork. During the summer months, and when schools are closed, the Tooth Taxi serves children spending time at their nonprofit community organizations. A Tooth Taxi Site Partner Request Form must be submitted to be considered for a Tooth Taxi visit. Request forms must come from an authorized representative from the school district or school; such as the principal, teacher, or school nurse.

SERVICES OFFERED BY THE TOOTH TAXI

The Tooth Taxi brings dental care right to children and young people by visiting schools and other community sites. Our licensed dental team assesses each child's oral health and provides comprehensive care. They also provide oral health education through classroom presentations, teaching young people how to prevent tooth decay, and how a healthy mouth can affect the entire body. The Tooth Taxi provides comprehensive dental care to any child who needs help.

Fill out the REQUEST FORM and submit it to the Tooth Taxi’s Project Manager.

When filling out your request form, be sure to choose a primary contact person at your site to help organize the visit. This should be someone who is able to commit the time needed to prepare for the Tooth Taxi visit. If your site is selected, the Tooth Taxi Program Manager will work with you to coordinate a successful visit.

Identify the patients you want served. We rely heavily on you to tell us who needs help the most! We give priority to youth who have no dental insurance, have never been to a dentist, or have an urgent need.

Upon booking confirmation, you’ll be given consent forms to send home with prospective patients. Collect and prepare Tooth Taxi forms in advance of the Tooth Taxi visit, making them available to the Tooth Taxi Program Manager.
